Comprehensive Care in One Place

On.Top is one of the only centers in Israel led jointly by a dermatologist and an oncologist under one roof. The center offers access to one of Israel’s top oncologists, Prof. Gal Markel, an expert in melanoma and all types of skin cancer.
Here you will receive comprehensive and personalized care, consultations, follow-up, and second opinions for patients at high risk or already diagnosed with melanoma or other skin cancers — all at the highest level.

Timely and precise treatment is critical for effective outcomes and preventing disease progression.

Initial Consultation

When dealing with melanoma or skin cancer, both an initial consultation with an oncologist and a second opinion can be crucial for making informed medical decisions.

At the time of diagnosis, there are often multiple potential treatment paths, each with its own pros and cons depending on the specific case.
The opinion of an experienced specialist can help ensure the most accurate treatment and improve the chances of a cure.
For those already undergoing treatment, a second opinion can offer better understanding of the condition, help evaluate the initial treatment recommendations, and compare different therapeutic approaches.
It’s also important for patients to feel comfortable with their oncologist and be able to ask questions and receive clear answers.
A second opinion can reinforce confidence in the proposed treatment and reduce uncertainty or anxiety.
During the consultation, the oncologist performs a full and thorough review of all medical information, including a clinical evaluation, to build a clear and up-to-date picture of the situation.

The session includes a comprehensive, clear, and direct explanation of treatment options — with support for both patients and their families — to help implement the most effective plan to manage the disease.

Prof. Markel’s main area of expertise is the treatment of patients with melanoma and other skin cancers.
When melanoma is detected early, the chances of a cure are high — but optimal management still requires expert oncological judgment for personalized treatment and follow-up planning.
Treatment of advanced melanoma requires in-depth knowledge of the disease and familiarity with cutting-edge biological and immunotherapy options.

Prof. Markel’s many years of intensive clinical experience allow him to provide top-level consultation, follow-up, and second opinions at any stage of the disease. He also treats patients diagnosed with non-melanoma skin cancers, including:

  • Squamous cell carcinoma (SCC)
  • Basal cell carcinoma (BCC)
  • Merkel cell carcinoma
  • Other rare skin tumors

In all cases, oncologic care is based on an individually tailored protocol aligned with the patient’s clinical status, with shared decision-making and close follow-up.

At our center, you’ll find a dedicated team of experts — experienced medical professionals focused on the diagnosis and treatment of melanoma and skin cancer, along with an administrative team to support you with all logistics and coordination.

When should I schedule an initial consultation?

Melanoma and skin cancers carry significant risks, so it’s crucial to consult with a specialist oncologist as early as the time of diagnosis.

Optimal treatment requires in-depth knowledge of these diseases and familiarity with the various modern treatment options and strategies.

An oncologist will not only use professional judgment to tailor the best treatment and follow-up plan, but also help you:

  • Understand your condition
  • Evaluate your risks and chances of recovery
  • Clarify uncertainties
  • Reduce fears
  • And most importantly, provide clear and comprehensive answers to all your questions.

Here are some helpful questions to consider during the session:

  • What is the stage of the disease, and how does it affect my chances of recovery?
  • Are there any additional tests I should do?
  • What are the recommended treatment options, and what are their potential side effects?
  • Are there any innovative or advanced treatments that may be suitable for me?
  • How can the disease be monitored during and after treatment?

The decision to seek a second opinion is personal and is recommended in any situation of uncertainty or concern — such as a new diagnosis, doubts about an existing diagnosis, reviewing initial treatment recommendations, or at any point during the course of the illness.

It is also useful when comparing different treatment approaches, clarifying risks and outcomes, exploring experimental therapies, or simply gaining greater confidence in the proposed plan.

A second opinion may offer a more accurate assessment and open up better treatment options. It is your right to seek a second opinion at any stage, in order to fully evaluate all the options available to you.

  • What is the stage of the disease, and how does it affect my chances of recovery?
  • Are there additional tests I should undergo?
  • What are the recommended treatment options, and what are the possible side effects?
  • Are there any innovative or advanced treatments that might suit my condition?
  • How can the disease be monitored during and after treatment?

A second opinion is exactly that — an expert’s additional perspective to expand your treatment options and strengthen your confidence in how melanoma or skin cancer is managed.

At On.Top, consultations are available with no obligation to proceed with treatment at our center.

As leaders and partners in every patient’s journey toward recovery, our medical team places your well-being above all else — whether you choose full 360° care with us or seek a one-time consultation.

To make the most of your consultation, it’s important to come prepared with all relevant medical information in your possession, such as:

  • All pathology reports related to your condition: skin biopsy, surgical reports, biopsies from other organs, etc.
  • A recent general medical summary, including a list of your regular medications
  • For a first-time consultation, bring the medical summary from the physician who diagnosed the skin cancer (e.g., dermatologist, plastic surgeon, general surgeon, etc.)
  • For a second opinion, bring all documentation and visit summaries from your treating oncologist or surgeon
  • Imaging results such as PET-CT, MRI, or others
  • If you have photographs of the tumor or lesion taken during follow-up, these can be helpful
  • Be prepared to share details about your family history of cancer

We also encourage you to bring a family member or friend with you for support.
